Dry eye treatment for clogged meibomian glands
Dry eye treatment for clogged meibomian glands
Burning, tired, or constantly watery eyes? Sometimes the problem lies with the lipid layer that protects the tear fluid. In suitable clients with meibomian gland dysfunction (MGD) can Meibomian IPL with the Lumenis M22 be an additional treatment.
I treat the suitable skin under the eyes and on the cheekbones, not the eyes themselves. During the consultation, I discuss your concerns, any ophthalmological diagnosis, and whether IPL is suitable for your skin and situation.
Important: Not all dry eyes are caused by meibomian gland dysfunction. IPL does not replace an examination or treatment by an eye care professional. If your symptoms have not yet been investigated, I recommend an appropriate ophthalmological assessment first.
Below you can read who Meibom IPL is intended for, how the treatment proceeds, what aftercare is needed, and what scientific research shows about this.
Who is Meibom IPL suitable for?
Meibomian IPL can be considered for suitable individuals with meibomian gland dysfunction (MGD). The meibomian glands in the eyelids produce an oily substance that helps prevent tear fluid from evaporating too quickly. If its release or composition is disrupted, the tear film can become unstable.
Symptoms that may be associated with this include burning, a gritty feeling, tired eyes, fluctuating vision, red eyelid margins, and sometimes excessive tearing. These symptoms can also have other causes. Therefore, an ophthalmological assessment is important before considering IPL as an additional treatment.
Rosacea and eyelid margin inflammation can coexist with MGD. I take redness, visible blood vessels, or sensitive skin into account during the assessment. If ocular rosacea is suspected, collaboration with an eye care professional is important.

How does IPL work for meibomian gland dysfunction?
IPL stands for Intense Pulsed Light. With the Lumenis M22, controlled light pulses are delivered to suitable skin zones under the eyes and on the cheekbones. The treatment focuses on the skin surrounding the eyes; the eyeball is not treated.
IPL is being investigated, among other reasons, due to possible effects on superficial blood vessels and inflammatory processes around the eyelids. For certain forms of evaporation-related dry eyes caused by MGD, it can be a supplement to standard care.
The precise mechanism of action has not yet been fully elucidated. Moreover, not every IPL treatment or combination of techniques is directly comparable to the treatments from scientific research.
How does a Meibomian IPL treatment work?
In advance: intake and assessment
I discuss how long your symptoms have been present, whether MGD has been diagnosed, which eye drops or other treatments you are using, and whether you have rosacea or eyelid margin problems. I also look at your skin type, medication, sun exposure, and possible contraindications for IPL.
Have your eye complaints not yet been investigated, or do they possibly indicate another condition? In that case, I recommend an examination by an optometrist or ophthalmologist first.
During the treatment
In the event of a suitable indication, I treat exclusively the appropriate skin under the eyes and at the level of the cheekbones with controlled light pulses. The eyes are protected with eye protection suitable for the equipment used and the treatment area.
I do not treat the eyeball, the movable eyelid, or directly against the lash line. The settings and treatment area are tailored to the skin and the applicable safety conditions.
Afterwards
I assess the skin reaction and provide personalized aftercare advice. After a series of treatments, we discuss whether you experience a change in your symptoms and whether a follow-up would be beneficial.
How many treatments are needed and when will I see results?
In clinical research, multiple IPL sessions are often used. The number of treatments and the intervals depend on the protocol used, the indication, and your skin.
After a series of treatments, we evaluate whether you experience improvement. Not everyone responds the same way, and I cannot guarantee a reduction in symptoms in advance. It is also not yet fully clear how much additional long-term benefit IPL offers on top of standard care.
We will only discuss any follow-up or maintenance treatments based on the outcome of the process.
What is the aftercare and what can you do yourself?
After IPL, the treated skin may be temporarily red, warm, or sensitive. You will receive personalized advice on gentle skincare, sun protection, and avoiding further skin irritation.
Warm compresses, appropriate eyelid hygiene, and recommended eye drops can remain part of your regular eye care. Which measures are useful depends on the cause of your symptoms and the advice of your eye care professional.
Do not change prescribed eye medication without consulting your healthcare provider.
When is Meibom IPL not suitable?
IPL is not suitable for every skin type or at every time. Recent intense sun exposure, tanned skin, certain photosensitizing medications, active skin infections, and other relevant medical conditions may be reasons to postpone or not perform the treatment.
For unclear or unexamined eye complaints, I recommend an ophthalmological assessment first. During the intake, I check whether treatment is responsible within the protocol and the safety conditions of the equipment used.
Acute eye problems? In the event of sudden vision loss, severe eye pain, pronounced redness of one eye, new flashes of light, or other acute symptoms, contact a doctor or eye care professional immediately. Do not wait for a dermatology appointment for this.
Frequently asked questions about Meibom IPL
Can IPL cure my dry eyes?
A guaranteed cure is not possible. With an appropriate indication, IPL may potentially reduce symptoms associated with MGD. It does not replace regular eye care.
Are my eyes themselves being treated?
No. I treat only suitable skin areas under the eyes and on the cheekbones. The eyes are carefully protected during the treatment.
Why do my eyes water when they are dry?
An irritated eye surface can produce extra reflex tears. If the protective lipid layer functions inadequately, the tear fluid can evaporate too quickly.
Can IPL also be used for ocular rosacea?
For some people with ocular rosacea and MGD, IPL may be considered as an additional treatment. An eye care professional assesses what ophthalmological care is needed.
Scientific substantiation
IPL has been investigated as an adjunctive treatment for dry eye symptoms associated with meibomian gland dysfunction. A systematic review of randomized studies found evidence of symptom reduction compared to placebo treatment.
Studies differ in equipment, treatment protocol, and any combination with meibomian gland expression. Therefore, results from a combined treatment cannot be directly extrapolated to IPL alone. Naturally, this has not yet been proven in extensive scientific studies – and therefore individual assessment remains important.

Do you suffer from dry, burning, or watery eyes, and do you have meibomian gland dysfunction? During a free consultation, I will assess your skin, any rosacea, and the possibilities of IPL as an additional treatment.
If you do not yet have an ophthalmological diagnosis, we will first discuss what assessment is required. This allows us to carefully determine whether Meibom IPL could be an appropriate step.
